2010/2011 Scholarship Application

The Cream City Medical Society, a chartered chapter of the National Medical
Association, has been serving the state of Wisconsin for over 80 years. We are a non-
profit organization comprised of African American physicians, podiatrists, pharmacists,
and other doctoral level health care providers. Our mission and focus is to mentor,
support, and aid in the development of new minority health care practitioners in the
greater Milwaukee area and beyond. Additionally, we strive to decrease health care
disparities withinin minority populations.

In keeping with our mission, we fund an annual Legacy Scholarship for deserving
African American students entering doctoral level fields in health care. Thank you for
dedicating yourself to the hard work of choosing to pursue a doctoral degree in the health
care field.

    along with this application. Applications that are incomplete will be returned
    to the applicant.

Eligibility Criteria

1.) Must be a student studying for a doctoral degree in the health profession
2.) Must have GPA of 3.5 or above
3.) Must provide 2 references (1 from an educational professional interaction, 1 from
    a personal interaction)
